I hereby grant my express, unconditional, voluntary and informed consent to SM Supermalls. (“Company”), its parent companies, subsidiaries and affiliates (collectively “Related Entities”), as well as its partners and its necessary third-party service providers (collectively “Service Providers”), if any, and hereby authorize them to collect, store, process, retain, update, and retrieve my personal information and sensitive personal information for the purpose of space inquiry submitted through the SM for MSMEs website

· Email Address: Provides a way for SM to reach you with important updates about your space inquiry.

· Mobile Number: Enables SM to contact you regarding your space inquiry.

The processing of said information may be made through the internal database, online, cloud storage and pursuant to the Company’s privacy policies. I was made aware of my rights as a Data Subject under the Data Privacy Act of 2012 and how to exercise the same. Further, I understand that I may access, update, or correct certain personal information, or withdraw consent to the use of any of my information as set out in this Data Consent Privacy Form at any given time by communicating with the Company, its Related Entities or Service Providers by sending a written request to scmc.dpo@smsupermalls.com.

As such, I also acknowledge and warrant that I have acquired the consent from all parties relevant to this consent and hold free and harmless and indemnify the Company, its Related Entities, and Service Providers to whom information was shared from any complaint, suit, or damages which I or any party may file or claim in relation to my consent.

The foregoing information shall be retained in our system for 3 years or as required by applicable law and regulations for legitimate business purpose, after which the system shall securely delete the data.
